Job Description

Purpose of Job Position

Identifying, treating, following-up on patient wounds, providing treatments on a day to day basis in accordance with physician's orders, and documenting.

Responsibilities and Duties

Wound Care

  • Perform treatments as directed by the physicians. Keep physicians informed of any changes in resident's skin conditions.
  • Complete Decubitus and Non-Decubitus Skin Condition Reports Weekly, Monthly, and Quarterly. Attend and present Skin Condition Reports at the weekly SOC meeting
  • Ensure all treatment orders written contain necessary information to provide clear instructions of how the treatments are to be performed.
  • Make weekly rounds with Wound Specialty Physicians.
  • Ensure Treatment Carts on each unit is stocked.
  • Manage and order "Part B Medicare" wound supplies.
  • Ensure Administration records are kept current.
  • Ensure treatments are performed in a timely manner.
  • Safety standards, written aseptic and isolation techniques are to be implemented and maintained at all times. Continue to implement and maintain programs for monitoring communicable and/or infectious diseases among residents and personnel and procedures for reporting hazardous conditions and/or

New Admissions

  • Perform skin assessment on all new residents and complete appropriate skin assessment sheet and scan into the resident's chart within 24 hours of admission.
  • Responsible for making skin condition recommendations for appropriate treatments and monitoring the progress.

Inventory Functions

  • Recommend the equipment and supplies needed of the assigned unit.
  • Assist with monitoring and developing adequate cost effective inventory control procedures.
  • Make periodic inspections of supplies and equipment to ensure adequate levels are maintained and equipment is available and working properly.
  • Implement procedures to ensure nursing personnel are trained and authorized to operate equipment in a safe manner.

Licensure/Certification Requirements

  • A Licensed Nurse in good standing and currently licensed by the State of Florida
  • Requires a Level II background screening with eligible status.
  • Must be able to successfully complete Drug Screening.
  • Subject to submission of Drug Screen upon hire, randomly, and directed.

Qualifications

  • Knowledgeable in cleaning, dressing and assessing the healing process of wounds.
  • Skilled in the development and implementation of appropriate treatment plans.
  • Comprehension and application of infection control techniques.
  • Knowledgeable in documentation of treatment plans and outcomes.
  • Excellent communication and interpersonal skills.
  • Previous experience with assessment and treatment of skin conditions commonly associated with Geriatric Residents.
  • Knowledgeable of Nursing, Medical practices and procedures, as well as laws, regulations and guidelines which pertain to Long Term Care.
  • Positive attitude with leadership and supervisory abilities.
  • Possess the ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies and procedures necessary for providing quality care.

Physical and Sensory Requirements

  • Must be able to move intermittently throughout the work day.
  • Must be able to read, write, speak and understand the English language.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility which include a medical and physical examination
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 4 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 10
  • Hours and days may be varied at times as necessary to maintain good nursing care.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Other Training, Skills and Experience

  • Ability to follow direction.
  • Ability to maintain professional relationships with residents and co-workers.
  • Demonstrate, on an ongoing basis, the ability safely operate motor vehicles.
  • Successfully compete facility required orientation and ongoing trainings as designated by the Administrator and Director of Nursing (DON) to include polices on Abuse Prohibition.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.

Summary of Occupational Exposures

Blood Borne Pathogens:

  • Tasks and procedures performed by employee involve risks classified by CDC as:
    Category I (Direct contact with blood or other bodily fluids to which universal precautions apply)
    Specific Task Exposure:
  • Resident Care: Blood, urine, feces, vomitus, wound exudates, semen/vaginal secretions, other body fluids- cerebrospinal, synovial, pleaural, peritoneal, pericardial, amniotic;
  • Handling Soiled Linens and Clothing: Blood, urine, feces, vomitus, wound exudates, semen/vaginal secretions, other body fluids- cerebrospinal, synovial, pleaural, peritoneal, pericardial, amniotic;
  • Resident Rooms and Common Areas- Blood, urine, feces, vomitus, wound exudates, semen/vaginal secretions. 
  • Refer to Exposure Control Plan for additional information.
  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.

Working Conditions:

  • Works indoors and outdoors in a variety of temperatures and conditions.
  • Moves frequently during working hours.
  • Work Surface varies from carpeting, linoleum, tile, concrete, gravel, dirt and grass.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, etc.
  • Communicates with the medical staff, nursing staff, and other department supervisors.
  • Works beyond normal duty hours, on weekends, and in other positions temporarily, when necessary.
  • Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Attends and participates in continuing educational programs.
