Job Coach, Individual Employment Services

SUMMARY

Join our team as a Job Coach under the supervision of the Director of Employment Services ! In this role, you’ll play a crucial part in carrying out evaluation and training objectives, ensuring quality, and promoting participant-driven placements that lead to customer satisfaction .

✨ ESSENTIAL FUNCTIONS ✨

• Monitor & Implement : Ensure proper implementation and documentation of Individual Service Plans (ISPs) for each participant.
• Program Coordination : Work with Employment Services Management to meet each participant’s goals in their ISP.
• Quality Assurance : Ensure program components meet the assessed needs of program participants, in line with their ISP.
• Compliance : Assist in maintaining compliance with Agency policies and CARF regulations .
• Crisis Management : Respond promptly and use intervention techniques during emergency situations.
• Timely Documentation : Ensure participant notes are completed on time and all required paperwork is submitted promptly.
• Liaison Role : Act as a liaison between the employer, program participants, and Momentum WORK, Inc.
• Petty Cash Management : Handle petty cash disbursements for program implementation.
• Team Collaboration : Attend required meetings and training sessions to stay updated.
• Additional Duties : Take on other responsibilities or special projects as needed.

PHYSICAL REQUIREMENTS 

• Ability to stand , sit , and walk for extended periods.
• Ability to go up and down stairs and assist with wheelchairs when necessary.
• Ability to lift up to 50 pounds using proper body mechanics.
• Full awareness of environmental stimuli.
• Ability to implement Crisis Management , Safety , and ProAct techniques (training provided).
• ADA accommodations available.

QUALIFICATIONS 

• High school diploma or equivalent required.
• 1+ years of experience working with persons with developmental and/or other disabilities preferred.
• Confidentiality : Ability to maintain the highest standards of confidentiality.
• Communication Skills : Excellent verbal and written communication skills.
• Ability to manage tasks with multiple interruptions .
• Compassion and respect for persons with disabilities, including those with intellectual , emotional , or behavioral challenges .
• Availability during disasters as per disaster plan.
• Reliable phone required.
• Valid CA Driver’s License with an acceptable driving record.
• Valid CPR/First Aid Certification (Training provided).
